£30,000 per year
Full-time role - 40 hours a week

Responsibilities:
• Coordinate care workers to customer visits using efficient rostering, ensuring continuity of care.
• Handle new referrals from private clients and social services, ensuring comprehensive care planning.
• Assist the Branch Manager in identifying further business opportunities.
• Assign care workers to customers while communicating all details regarding the assignment.
Requirements:
• Minimum of 3-6 months care coordinator experience in a domiciliary care setting.
• At least 12 months of domiciliary care experience within the last 3 years.
• Level 3 NVQ in Health and Social Care (or willing to sign up upon appointment, working towards Level 4).
• Knowledge of PASS and Care Planning systems (desirable).
• Basic ICT skills and familiarity with the 5 Key Lines of Enquiry (desirable).
• Driving license and own vehicle required.
• On-call duty every other weekend.
