Provide support and rehabilitation on a one-to-one basis
Provide person centred care within a multi-disciplinary team
Create, deliver and evaluate individualised care plans to meet service users needs.
Provide effective services across multiple sites for adults residing in community rehabilitations services
Responsible for a team of Occupational Therapy Assistants
Provide support and professional advice to other members of the team
Qualifications and Requirements:
The successful candidate will be able to work proactively both autonomously and in a team
Occupational Therapist degree
Member of the British Association of Occupational Therapy
Ideally you would have a specific interest in LD and Mental Health
